It may not make sense as far as where the ball went afterwards but in every shot I've seen the ball is absolutely coming out before his arm starts going forward. Heck, I've seen QBs lose their grip on the ball, then clench their fists while trying to throw it, and push the thing forward with a closed fist. The ball ended up going forward because after it came out of his hand the finger tips pushed it forward.
