  5. i look at MD as a phD level + training PA/NP are effectively masters level programs + training is there a difference in care? for 85%+ of the patients it’s not a difference when you have typical well known medical conditions. the bigger problems comes when you have more complexities in the case or the case diesnt fit the text book. i see specialists in two different locations. A couple of them are PAs because these are well controlled and maintain conditions but I want to still see them for awareness of the complexity of my conditions. i regular go to university medical centers. These are very different than typical medicine. Depending on the area theyare Nita’s focus on things like rd run and per hour patient encounter counts. In some areas the dr might have to have 4 patients an hour. university MC are heavy on med students and residency. This is different than a resident clinic which also exist. I was in a resident clinic previously. In some of the docs I see I do see a resident first. I do spend time with the md. Over my years I’m certain I’ve been written about in papers or talked about in talks given. pre Covid a well respected nationally known doctor from buffalo died. The national journal where he served as editor on had devoted part of an issue to him after his passing. I was easily in his top 3 complex/ experience/story about patients he ever saw in his career. If I got frequent flier miles for medical documents at childrens I would have circled the globe a few times.
  6. im talking in generalities. I’m not talking in very specific terms. Some of the staff is required to be on because it has an ICU. If the ICU was closed they would not be on staff in the hospital. These generally are for 2nd and overnight shift times. I’ve dealt in hospital staffing/data for a good chunk of my career. most do not. they really don’t know the difference between an MD and NP/PA. whrn it comes to communication if they are talked to by a nurse or some other qualified med tech thry might listen to them better than be intimidated by an MD.
  7. ICU care can occur when… 1. post surgery 24/7 observation 2. other conditions that require 24/7 observation because of potential medical risks 3. post infection observation 4. Covid and other diseases that might require immediate care hospital staffing is tied to ICU. Certain departments have a staff member 24/7 where their first responsibility is to an ICU call. ive been in the ICU 3 times as an adult. One post surgery fir 24 hrs and the other two times for a perforated lung.

When throwing the football generally the motion and strains are very different.. 1. Wrist strain is different 2. elbow strain is different. The elbow strain also involves twists and turns. You don’t get these whrn throw8ng a football 3. affection the shoulder is very different.

FACZt…8wont be in a few years when you have to pay $25+ a month fir all these subscription streaming services. has sports popularity changed? There might be some shifting between sports. I don’t think the market has changed. remember customer loses can come from 1. person moves from own home to senior / retirement home where cable is provided. They are not a direct customer 2 you have the creation of couples/ roommates from single. It could come as a lost customer even though it’s not 3 people died 4 people switch providers 5 people go to streaming from cutting the cord 6 kids still living at home in their 20s so they aren’t adding as customers to cable or streaming counts. 7 newer apartments come with built in cable as part of rent. the issue has been on cable sports channels is what can we run during the daytime to get viewers. This is why nbc sports died and FOX sports hasn’t done well either. idont watch sports talk shows during the day on espn. I do watch live sports on them I don’t watch streaming sports online. I’ve watched things like Netflix on my TV.

  16. the krngsington cut through and divided a black neighborhood. This is something that was commonly done throughout citues building expressways thru cities. About every one was racially motivated. Whites were more powerful and had control so they didn’t want it in their neighborhood. The wanted it thrus poor and minority neighborhoods. suburban built up also discriminated against blacks. Homr loans discriminated against blacks. Ever hear of “redlining”?
  17. You have many inaccuracies…. there was redlining and black zones. The Kensington expressway split a black neighborhood. This has happened in many other cities when expressways were built. They avoided white neighborhoods and cut thru black ones. the post WW2 GI bill benefited white veteran dips only allowing them to buy homes in the suburbs.   19. two stories…. 1 I played pick up hockey at the old sabreland off Niagara Falls blvd with some friends. We rented ice time for once a week for most of the year. I was on the ice and I make a very good defensive play sliding on the ice on my stomach. I get on my knees sliding on knee/ shin guards and I turn my body as I’m still sliding so my skate blades hit the boards first. I get up I can’t put any weight on my one ankle. I strained my Achilles. It took 6-8 weeks to get better on its own. I could walk and stand but could not strain it more with running or skating or peddling a bike. 2. a couple years ago I had a meniscus tear. I was scheduled to have surgery. It had to get postponed at the last minute because the surgeon tore his Achilles playing basket ball. You think the dr should know better…
  20. not if they do it like week 2 for this year with games split between abc and espn. this year it’s 7 espn, 830 abc but they could do 8 abc, 930 pm espn west game dont forget in 23 Christmas and new years are on Monday.
  21. to me in buffalo this housing market is insane the last couple years. the last housing boom of 2004-2008 skipped over buffalo. im curious who the buyers are…. 1. Are they true local home buyers 2. are they people who work out of nyc/ Boston/dc decide to work remotely from buffalo. It’s an easy 1 hrflight if they need to go into work. 3. high cost cities having folks sell their home and move to buffalo pocketing 50% of their old house sale for retirement 4. pure investors this is true with all secondary markets out there. the housing price value does not coincide with regular workers income. My rule on house buying is going to under 3.5 your household income at the start of a 30 yr mortgage for new home buyers. If it’s above that it’s going to seriously strain you financially. If a couples combined income is $125K you can’t low $$400K depending on down payment amount. ilook at the home I grew up in. the value generally fluctuated between $65K-$80K for years. My parents bought in in mid 60s. Ehrn my dad entered nursing home we had to sell it for a loss. It needed work. it was bought once as a fix up. It was sold again as a second fix up. I guess the first didn’t have skills to do some of the work. It then sold for about double its historic avg value. Thsts why I don’t see it retaining that value long term given its basically a starter home.

  23. For those looking to go to Baltimore above are two maps. you dont have to stay in downtown baltimore. The inner harbor area is fine The Baltimore aquarium is one of the best in the country. In the inner harbor the have various self boat rentals like pedal boats. Ft mcHrnry is where the anthem was written by francis Scott key. east side of inner harbor is little italy. if you stay at a bwi hotel…you take hotel shuttle to airport then take the free shuttle bus to the rail station. You can fly to dc airports and go metro subway to union station then MARC train to Baltimore. I don’t recall exact fare but it’s around $10 or so one way.
