  4. What about Colorado? The State Government isn't a mess, the taxes are not unreasonable, and overall is a pretty darn great place to live, even with all the Californians that have migrated here. Heck in my neighborhood, homes are selling before/ the day they are listed because Denver is doing so well- I am considering finishing off my basement and selling soon because there is such little inventory, its a great oppoortunity. I think it all depends on what you consider freedom. If you like Government services, then you live in a Blue area, pay taxes and enjoy life- aka Denver. If you don't like Government Services, and you ant less intrusion, regulations and laws, live in Colorado Springs... some people like low taxes and having th street lights turned off because they money ran out, its just preference. In all honesty, of course the Dakotas are free compared to other states.... nobody really want to live there... there is little demand/need for too much Government. California is full or retards who thought they can boom forever and their housing will never go down, so voted yes for everything and kept chugging... until it doesn't go on forever, and there is not enough money to pay for everything.. My autn finally sold her company because she was taxed to death, sued by emplloyees more that once, and had big losses when the ports shut down... she's finally relizing the beach ain't worth it...

I think the Medical Home will be enhanced by innovation is EMR's... the days of having to get records faxed or sent via mail, or home care orders being signed by faxed and sent shou;d become a thing of the past.... its interesting to see my place operate under an all incompasing EMR versus when I got here and it was electronic bilings and registration, but charts and ordering were still hardcopy and shadow charts... tech will be the tool that allows that provider to deliver quality cost effectively to that 2500 patient panel... are you an internist?

  7. The article has some good points, but I think it off target on a few others: 1. Physcian's are not unlike other humans, they work hard only equal the incentive. If they are salaried, and need to produce the RVU's to cover salary and benfits, they will figure out down to the single digit the minimum number they need to see to cover that, and will simple cut clinic time if they meet the benchmark. I personally believe they shoudl be incented to see as many patients as humanly possible, but make sure their reimbursement is conmmensurate with their patient's outcomes... I think that is one thing they are trying to so by getting clinics certfied as PCMH (Patient Centered Medical Homes, or some **** like that)... they want to deliver the best care by collecting and evaluting measureable such as diabetes care.... but it doesn't necessarily incent providers to see more patients if the financial incentive is not there 2. We have acquired 3 smaller hospital in Colorado, one in the Springs and two in Fort Collins. Small Medical practices will not be able to compete int he future, unless you open a concierge practice and market to high value individuals and families... however, wealthy pateints are not dumb.... they know when you get sick you need the Medical Center for treatments, especially Cancer... so many of them are making donation to the foundations of Academic Health Systems and establishing care with the internist who handles the VIP's... At least on Colorado, we have 2 players in the Health Care game, one Academic on Private.... Good article overall, but the ACA did not all of the sudden bring on these symptoms, they have been in place for decades...

  10. Its why most Americans don't identify with the Govt Spending and Public Debt issue in this country. They keep hearing Medicare, SS will not be touched, ever, and they associated that with their own lives and that there don't have to be tough choices to be made.... its makes sense when you think about it, most Amercians do not fear debt, they relish it- and those who don't relish it at least see it preferable to say "no" to wingin out the Discover Card... My in-laws are always cryig that the have no money, they can't retire.. boo-hoo, whoa is them.... but they just bought a $2,000 headboard for their bed.. not in cash either, bet you... not only is that something no one needs, their son-in-law builds custom furniture and would have done it for materials costs... you talk about !@#$ing stupid... sheesh... I can go into almost any American household, organize their expenses (they don't do it themselves, guaranteed) and cut 20-30% wihtout them starving or bleeding out of their eyes... ohm there would be bitching and screaming about not being able to watch Honey Boo-Boo and craking up that stove and oven, but I'll bet I could do it, easily, if not cut more... You may be asking, "Does B think he knows what is best for others?" The answer: Yes, yes I do...
  11. I am often surprised when I run into so many retarded Americans with regards to money. I just heard a commercials about a Credit Union that is urging people to switch the credit card balance to a lower rate, to get a lower monthly payment on holiday spending...
  12. they could go elsewhere, but why? Most only work 3-4 1/2 sessions per week, get at least 1/2- 1 full day adminstrattive time (in which some consider a half day off)- heck clinic sessions are short too, 4 hours in the morning and 3 in the afternoon... most of them after a few years are doign research and barely see patients... an easy 8-4, compared to a speciilist of surgeon working 60-80 hours a week. They can get bonused if they hit certain RVU benchmarks over what covers their salary and benefits, but the bonus is not mind blowing considering how many RVU's you have to generate to get them. Many leave thinking the grass is greener, but end up workgin alot harder, paying their own E&O, and come back... plus the Division of Internal Medicine is a money loser here, they are subsidized essntially by specialities and surgical..
  13. I am still not sure how that helps the system.. Insurers will be regulated by someone, do people really want it to be regulated by the Federal Government versus State Governments? And Mandate how people live, and penalize them... don't isurers already do that via Rating? But I do like the HSA and Cat insurance much better that HMO plans. Heck, with more customers, better reimbursements- our internists are still making 150K on average, that still darn good change.. now most of them are outlining Research projects to reduce their clnical time, but that might just be because seeing sick people all day beats you down... and its way better to get a grant and study how many patient visits is takes to justofy 1/2 day of admin time, than to diagnosis and treat people...

I shoud have been more specific Thank you B! yep, 36 and not getting any younger... and most on this board perhaps argue none the wiser.... lol Its interesting, I went a private catholic high school because I was bordering on retardation leaving 8th grade, and my parents had the money and determination to get me straigtened out. What I found very interesting about the school was there were ALOT of young men who came from relatively poor families, but it was importan to them to get their kids the best education possible. I have to say the difference in expectations and rigor from the Williamsville Public Schools (considered pretty good) to St. Joe's was very noticable. Maybe that was because I was an idiot and needed to do some serious work to get up to speed, and perhaps kids in Public School who applied themselves sought our rigor and pulled out an excellent education nonetheless, but I will say I believe the private education was better overall at delivering excellent young men, not just graduated students. People seem to get hung up on the Religious piece of the schooling, but I have to say, I never felt unfomrtable with it. It was not forced on you, in fact I did not attend liturgies by choice and they were supportive. Whats at the core is parents want an excellent education, they are already shelling out big bucks in taxes, why not at least try if a different way? If the reulst sucks and it costs way more, then change the laws... but to no even try, wasted opportnity...
  22. MSNBC is horrific. The Al Sharpton show on there is awful, he sounds like he had a stroke and sturggles with words an thoughts... The Chris Matthres Hard Ball is nothing but partisan hackery. I actually ask the gusy at the gym to switch to Fox so I can watch The Five. Its partisan, but not in the way MSNBC is... although one of the guys on The Five said the dumbest thing a few weeks ago, when Chavez died, he said "thats poor reflection on Cuba and theie National Helath Model"... how dumb was that, at the time they had no idea how he died and from what... partisan hackery at its best! Lets be honest, I watch it mostly because of the hot chicks...

The GOP will never be Pro Gay Marriage, that's why the Libertarian's have been able to gain traction with in the party, and support by more people The GOP will not support a massive overhaul of Education System, they already did that with NCLB to at best mxed results, if anyting they would support dismantling an Federal involvement in education, and leave it to State to decided is school choice is their flavor, of the tradtional public school system. To me, Romney often looked like he was tryign to hard, it wasn't fluid for him to be a campainger. One night he looked on, one night he seemed comfortable, but unfortunately its a marathon and not a sprint.
  25. The biggest challenge for the GOP??? Communicating their ideas and principles in short, feel good snippets that ring true in the heads of people with minute attention spans.... If you think about it, Conservatism is about preserving traditional limited scope of Federal Power, or, reducing that scope. Most Amercians have only known the Government we have today, the big one with its hand in almost every sector, seciton and seat in American Life... there is belief that Government is the Lever in which we solve problems in this country... in some sense, Government is the preferred solution tool (in a way, Conventioned Wisdom might say a Central Planning method is most effective, althought empirically we know that is not true)... so it is really hard to convince a population of people who are unsure that less, cutting, smaller services who already have a short attention span that less is better... its not a sexy message, its practical and asks people for sacrifice.... we don't like to sacrifice, its why we put everyitng on credit, why can't we have it all... Democrats have it easier, their message is one of hope and programs and services and expansions of benefits to the old, poor, slow, discriminated against, etc.... it feels good to most people to want to help those people with Programs... so the message is we have the answers, vote for us and we got your back... Republicans can stick to thier principles, but they need to convince Americans that less will in turn produce more... good luck with that, what time is American Idol on? What were we talking about, anyway....
