  1. You want to fix income disparity? (I refuse to use inequality, income have never been equal or "fair") - Roll back time to place before Globalization of Commerce. One of my patients started a company that helps Medical Offices billing claims- they're biggest cost saver is routing all claim related processes to India over the wire for coding, verification etc.- Globalization puts downward pressure on incomes in developed nations. - Stop using Automation- get rid of ATM, gid Rid of Amazon, Get rid of all cell phones, computers, etc all over the world- if we rolled back progress 60 years, more people would be doing more job that machines. - Cut the US population by half- all of the sudden there woudl be wage increase and corporate fights over a worker wiht a brain. See, easy fixes. Its the new reality, folks, and no minimum wage increase or legislation will stop the forces of change. There will be a point were incomes no longer cover the essential basics, credit will be maxed out, and the system will fall apart... then and only then will we see emergence of a new normal.
  2. I almost thought you were talking about the Stimulus. I caught up with a friend of mine who owns a concrete/paving supply company, hadn't seen him in years.... I asked him about business, and we got talking about stimulus. He said it was a boon for him, but much of the money was wasted hastily. The City of Denver got a boatload of money, but with the stipulation it had to be used immediately. So instead of doing a few long term worthwhile projects, the skimmed 2 inches of ashpalt off of ever street in the city and surfaced paved... He said most streets needed actually repair to streets, water pipes, etc.... He said in a few years most of those paved streets will see that new paving starting to break up, or dug up for real repair at taxpayer expense. Just because the money is appropriated, doesn't mean you have to open the flood gates... No pun intended

I will disagree on Healthcare with Dr No until time ends most likely, but the country will miss this practical government steward. A guy who truly embodied the notion that Government should have a very compelling reason to interfere in the lives of private citizens, that no amount of activity nor expenditure is sacred and should go without examination. Was be perfect, no.... But you have to believe if all reps, senators and POTUS' were cut from the Coburn cloth, we wouldn't be nearly as bad off staring down the Barrell in the USA...   5. you have to be specific when posing the question, and in my mind there are 3 types "Government Funded" healthcare: 1. Socialized Medicine- the Government run the Hospitals, employs the Practicioners, collects taxes foe the cost of care. 2. Single Payor- the Government collects taxes to pay for care, people use private providers. 3. Subsidized Premium Support- Private system the government collect taxes and subsidizes premium payment for private health insurance- they do not pay claims, and don't own the means or care. We doubled down on a system that really didn't work in the first place if you were not healthly, not young, had preexisting condition, etc. We fixed access, but did little to address cost of care in the grand scheme of things. Yes, there will be cost cutting measures and maybe those will help, but the key disconnect is that individuals/ hospitals do not have an efficient cost/ payment relationship because of third party payments. Its still almost impossible to get a cash quote for services at a hospital in this country... I'd like to the States/Federal Government fund a risk pool that pays claims that are over a certain amont based on each person income. So the individual is charged with paying for their routine care, seeking out cost effective ways to do so... when they need the 50K surgery, the risk pool pays the provider. I'd like to see a system like this have an auto opt-in... so people would have to make a choice not to participate/opt out, so either go without coverage for which they could be denied care if needed and couldnt pay, or buy a private plan if they qualify. Fund the risk pool with taxes, each person has a deducitble and an HSA they could use if they wanted. You take TPA out of the mix, get a better relationship between consumer/ provider and get economies of scale in the process. The people of the Nation/ State could vote on how much they wanted to spend for care in the country, instead of now it seems like a open spigot free for all. People vote for no waits for anything, more spending. Americans decided a hip replacement wait of 6 months is acceptable, less spending. Seems reasonable to me.

AWESOME but seriosly, my kids would be in private school- there was never a wasted moment in my private schooling, always nose to grind stone- and the teachers didnt make ****, they just loved the place because they had motivated students who had parents who cared.

I have found almost all like plans I have compared from my place of employment versus the Colorado exchanges to be about the same end cost in premium when you correct for the benefit credit I get from my employer. essenitally, my plan here and the plan on the exchange, the managed care bronze, like deductibles, is about $350-$400/mo. I assume Walmart $40 premium cost to the employee has a large amoung paid by the employer... I had nurse working for me that was being actively treated for Esophogael cancer, couldn't afford not to....there is another girl here in Radiology that is getting treatment for Breast Cancer for the 3rd time and hasnt missed a day of work. the Chronically sick work all the time.

Obamacare is not a medical plan, its not even what the actualy law is called. The largest employers in the US has a group plan that is less expensive that individual plans on ACA exchanges... wow, what a revelation.

  14. why would a national self contained managed care company go away? Personally, KP does a great job at being efficient and cost effective for less than serious issues, but it would not be my provider of choice. I have a ton of KP subscribers who come here self pay for Spine Issues, Cancer TX, and other specialties like Rheum and Neuro- althought KP will autorize out of network work-ups if they don't have an expert in the particular condition. I think you will see more Kaiser model copycats on the local and regional level. We will be offering insurance coverage that is adminstered by the hospital, and we have acquired locations in Fort Collins, Colorado Springs and soon the mountains to help accomodate the Front Range in Colorado.

  17. We also couldn't do much for people either.... expensive medical innovation and cutting edge tech has drive up cost/prices, we can also treatment almost everything. But yes, TPA and the "claims" process is a trainwreck... glad we doubled down on it.

I do however wish everyone a happy and healthy 2014 regardless of politics- my wife and I have to much to be thankful for, and 2013 was excellent to us both my choice and by a little luck. 2014 is shaping to be a year where we make tremndous strides in happiness, may you all get the same.

  23. Howard Dean is a retard. the Individual Mandate is the Sun to the ACA's Univserse.... the private insurance system doesn't work unless the payors balance out the claimers.... If anything this is an underhanded way of saying "its stratgeic failure, we really want Single Payor" You passed the convuluted, inneficient ACA private insurance plan scheme Dean, you make it work now. Own it, I am tired of it.
